America has spent decades being told the sun is basically a nuclear death orb, and that sunscreen is the responsible adult in the room.

The data doesn’t agree.

Melanoma has not disappeared. It has climbed hard. The CDC reported that U.S. melanoma rates doubled from 1982 to 2011. The Skin Cancer Foundation now estimates annual invasive melanoma diagnoses rose 46.6% from 2016 to 2026.

Something is not adding up.

What is sunscreen good for?

Sunscreen is useful. It reduces UV damage, helps prevent burns, and there is randomized trial evidence from Australia showing regular sunscreen use lowered melanoma incidence over long follow-up (Journal of Clinical Oncology).

It also helps skin aging. In a 4.5-year randomized trial, daily sunscreen users had 24% less visible skin aging than discretionary users.

So no, this is not a "throw sunscreen in the trash" newsletter.

Melanoma is more complicated

The sunscreen and melanoma story is less settled than the public health slogans make it sound

There are three major skin cancers, basal cell carcinoma, squamous cell carcinoma, and melanoma, and the distinction matters: basal and squamous cell cancers are far more common, but melanoma is the one people worry about most because it is much more likely to spread and become deadly when caught late.

In a 2018 systematic review and meta-analysis covering 29 studies and more than 300,000 participants, researchers found that sunscreen use was not significantly associated with lower melanoma risk; across 25 melanoma studies, the pooled result was essentially null.

Sunscreen does seem to prevent other types of skin cancer, especially squamous cell carcinoma, where randomized trials show clearer benefit, but the melanoma evidence is far less clean.

Avoiding the sun is worse than smoking

In a large Swedish cohort of nearly 30,000 women followed for about 20 years, women who avoided sun exposure had shorter life expectancy than women with the highest sun exposure.

The authors wrote that nonsmokers who avoided the sun had a life expectancy similar to smokers in the highest sun exposure group (Journal of Internal Medicine).

Sunlight helps regulate circadian rhythm. UVB exposure drives vitamin D production in skin. Outdoor time tends to come bundled with movement, better sleep timing, and less couch rot. Good luck monetizing that.

The SOS sun protocol

  • Get regular outdoor light early in the day. This is for circadian rhythm, not tanning.

  • Build exposure gradually. First sunny trip of the year is where people do dumb things.

  • Never burn. A tan is not the goal. A burn is a receipt for losing.

  • Use mineral sunscreen when your latitude and genetics do not match. Think Irish skin in Miami, Scandinavian skin in Mexico, office skin on a ski mountain.

  • Prefer zinc oxide or titanium dioxide. The FDA says those two have enough safety data to be considered generally recognized as safe and effective (FDA).

  • Be more careful near water, snow, altitude, and the equator. Reflected UV is rude.

  • Use hats, shade, and clothing when the UV index is high. This is not cowardice. This is not being cooked.

  • Get suspicious moles checked. Early localized melanoma has extremely high survival, while late melanoma is a different game (SEER).
